Then an MOC changes the purge time, the SOP never gets revised, and the crews drift — the exact finding investigators write after the worst day. CAIRN reads the controlled set the way an auditor would: a required action with no completion evidence is OPEN — never assumed done.

The MOC card
The shelf holds cards sealed from the controlled set: MOC-2026-018, whose follow-up to revise SOP-300-12 shows no completion evidence — status OPEN, quoted verbatim; and the SOP's own card with every numeric limit carried exactly, units and all. Counts and receipts are live.
The unit speaks
The SOP says thirty minutes. The approved MOC says fifteen. Ask the document
set what the purge time is — and watch a grounded answer come back with [n]
citations to both documents, the conflict visible instead of averaged away. This is the
question a crew asks at 2 AM.
Two controlled documents disagree
Pre-baked passage vectors make the sweep live and model-free: similar-passage pairs surface as candidates, and pairs where both documents are controlled are flagged as the worst kind — the exact condition a PSM signer certifies against. Watch the SOP and the MOC find each other.
The clause that does not exist
